Membership Forms

Association forms are available here for members' convenience. All forms are Adobe Acrobat files that can be downloaded, printed, completed, and returned Tim Fillmore. If you do not have Acrobat, you can download the free reader from Adobe.

Application (44K) - Filling out an application is the first step to becoming a member of Pacific Baseball Umpires. By filling out a membership application you can give us an idea of your umpiring experience, training, and goals. Please print this form two-sided on a single sheet of paper and mail to Tim Fillmore at the address on the form.

After you have submitted an application and are accepted as a member, you will need to fill out and return the following forms and pay membership dues in order to complete your enrollment.

Member Information (70K) - This form provides basic member contact information needed to add you to our roster . Please print this form and mail to Tim Fillmore at the address on the form.

Release Form (114K) - This is a standard release form which must be on file before you can be assigned games or participate in training clinics.? The second page of this form is the CIF San Diego Section "Ethics in Sport Policy" which must be signed by all officials prior to working any CIF sanctioned games. Please print this form two-sided on a single sheet of paper and mail to Marc Rico at the address on the form.
